1. Open a new document of the size that suits your needs. Using the Rectangular Marquee Tool make a selection to create the Book Cover. I wanted my Book Cover to have the metal texture, but you can choose some other texture. As a reference, I used excellent tutorials by Jezebel and by  Yves Allaire. From these tutorials you can learn more than how to make this metal sheet. Here is how my metal sheet looks. 

2. You can make your own texture for the antique paper, or you can go to Google  and search for "texture". You can also download the one that I used  here  by going to Files: Free downloads.

3. Now we have to make the displacement map that will give the form to the pages of the book. Open the new document of the same size and fill it with 50% gray. With the soft Airbrush make a vertical stroke near the center of the document as shown, and apply Filter> Gaussian Blur>Radius 3px. Save the document as the PSD file and name it "Left page Displacement map.psd".

4. Go to: Filter > Distort > Displace. Horizontal:8% Vertical: 8% , Stretch to fit:checked. Note: Value for distortion depend on the size of your document.
Click OK, new window will appear  to chose a PSD file. Select the PSD file that you created and saved as Left page Displacement map. This is how your image should look .
Tip: To Displace Right side of the page  you do not need to make a new Displacement map, just use Transform Tool and Flip Horizontal  and Save as Right page Displacement map.

5. Using Rectangular Marquee Tool make a selection of the left page side and copy and paste to a New Layer (Ctrl+J). How many copies you will need depends how tick you would like your book to be. I made five copies and nudge each to the right by few px. Make  same numbers of copies for the right side and nudge this time to the left each page for few px, as well.

6. Add to the pages shadows and highlights. This is how your image should look. From here you can add your personal touches.
